The Incident Review Process is a procedure for addressing concerns regarding a specific incident. It is intended to help midwives improve their practices and improve future outcomes.

An Incident Review is initiated in two different ways:

  1. Midwives self-report out-of-hospital births with any of the following clinical indicators within 14 days:
    • Maternal/Neonatal Mortality
    • Maternal Shock
    • Uterine Rupture
    • Maternal/Neonatal Seizure
    • Uterine Inversion
    • NICU admissions within 72 hours of birth (except for observation/congenital anomalies)

  2. Any individual may make a complaint about a Professional MAWS member using the Complaint Form.

If the incident meets the criteria for review, a peer group is assembled in accordance with the Quality Management Program Document.

The Peer Review Process is a routine retrospective educational review and prospective evaluation by professionals of similar training. These peer reviews are conducted at the regional level and primarily coordinated by the Regional Representatives. MAWS is committed to providing other routes to obtain peer review for those who do not have or are unable to attend regional meetings. All-region peer reviews will generally be held bi-annually often preceding MAWS conferences. Contact Darlene Curtis, LM CPM for more information at CMBC1999@yahoo.com or 425-330-7249.

It is the responsibility of all professional MAWS members to review fellow midwives and to be reviewed at least every 2 years. Each Peer Review will consist of at least 3 professional MAWS members in addition to the reviewee. The peer review is a confidential process with confidentiality statements signed by all participants.
